New Patient Center

Thank you for your interest in becoming part of the family here at Pediatric & Adolescent Center of NW Houston.  Prepare for your appointment by following these instructions.

BEFORE YOUR APPOINTMENT

Insurance Information
Please ensure we have your insurance information prior to your appointment date. See our Billing & Insurance Page for a list of accepted insurances. Our staff will make every attempt to verify your benefits prior to your appointment. Newborns: Please be advised, most insurances ask that you enroll your newborn within thirty days of his or her date of birth.

Medical Records
Newborns: When you and your new baby are discharged from the hospital, you will be provided with discharge paperwork and possibly a shot record listing your baby's first Hepatitis B vaccine. Please bring these documents with you to your first appointment. Transfer Patients: If your child is transferring to our practice, you may prefer for us to have a copy of their records. To transfer your child's records, please complete our Medical Release Form and provide it to your child's previous physician. This form will give your previous physician permission to either provide your child's records to you or fax them directly to us. If you choose to have your child's records provided to you, please bring them to your first appointment for us to copy.

New Patient Paperwork
Please print and complete our New Patient Paperwork to expedite your check-in process on the day of your first appointment. If you are unable to print and complete the New Patient Paperwork, please arrive to your appointment twenty minutes early.
